Engineers begin their design endeavor by answering “what might work and what can go wrong”. Engineering designs are successful only to the extent that designers foresee and understand how a system may fail to perform its necessary functions. When designing a bridge, the engineer needs to know what loads could challenge the strength of the individual steel members. When designing a communication system, the engineer needs to predict what might cause connection problems within the communication network. When designing a steam boiler, the engineer needs to anticipate, for example, the failure scenarios that would build up pressure and cause a boiler explosion. The concept of risk combines chance for failure with the consequence caused by the failure. An essential element of risk is the uncertainty – the fact that engineers don’t know exactly what failures will occur and when and where the failures will occur.
